    Inexperienced and inattentive crewmenbers in both seats has been the driving force in a lot of episodes here as well as other places. Luckily none of the events has gotten people hurt yet.
    A Fed on my jumpseat recently, told us several stories that would make you cringe. Luckily none of those events resulted in people getting hurt but did result in some form of certificate action.

    Again, one bugging the wrong heading is one thing and yes, an error in and of itself, but the PF mindlessly flying an incorrect heading bug, especially in the opppisite direction into traffic......well.

    Very likely an unstable approach was a component. Why, is the question. As the regional airline industry is forced to scrape more metal off the bottom of the barrel in pilot aptitude and qualifications, situations like this will only increase. Had this plane slid more sideways and cartwheeled, it would no doubt be a much more tragic story.

    Just a matter of time, if you ask me. The question there is, who will it be ? You can be sure pilot qualifications and training history will be a major focus for investigators.
