Messerschmitt Me 262 "Schwalbe" - the world's first turbojet fighter aircraft, which first flew in 1941 with piston engines, and 5 years later with jet engines. The only two countries that used this plane were Germany and Czechoslovakia respectively, the Czechs retired these planes in 1957.<br />

Aero Commander Shrike - In the cockpit with Bob Hoover at the controls. In a barrel roll, there are no forces acting on the plane (If performed correctly), of both acceleration or gravitational load forces. That is why the iced tea was able to be poured, even though the plane was upside down. An Amazing pilot and video!<br />
<br />
F-14 Tomcat - Performing a high speed, low altitude pass of a carrier. The speed was probably around 550 - 600 knots, or 990 to 1080 km/h, subsonic (that was not a sonic boom) but still producing a very loud (120dB+) sound. Look at the water!<br />
<br />
Grumman S-2 Tracker - it barely has enough speed to rotate, let alone cope with all the drag obtained from flying into the wave, but it still manages to climb out.<br />
<br />
F-14 Tomcat - Another Tomcat performing a high speed, low altitude pass of a carrier. The speed was probably closer to 650 knots, or 1170 km/h, barely subsonic (that was not a sonic boom), probably even transonic, if you notice the large cloud forming, called the Prandtl-Glauert singularity.<br />

Boeing 707 - Filmed during its flight tests. The pilot, Tex Johnson, performed during this flight a barrel roll, much to the dismay of his employers. However, the military were viewing the flight, and they were so impressed at the manoeuvring capabilities of the passenger jet, and signed a contract with Boeing to produce military aircraft. Tex was not fired for his stunt, just asked not to do it again.<br />

McDonnell Douglas RAF GR7 Harrier - The Harrier in High speed flight, very widely distributed video of it passing just meters over the heads of the military personnel on the ground.<br />

			<p>TMZ has obtained an explosive videotape -- well, a prematurely explosive videotape -- showing the extremely graphic and dangerous activities inside the chopper of celeb pilot David Martz... and let's just say they don't call it the cockpit for nothin'.<br />

The tape proves Martz -- who's currently being investigated by the FAA -- was undeniably distracted by his porn star passenger Puma Swede ... for 47.5 seconds anyway ... while his chopper was flying high above San Diego and his pants were flying down around his ankles. And there were many minutes of foreplay, where at least one hand -- in particular, several fingers -- was definitely not on the controls.<br />
<br />
While Martz recklessly buzzed over crowded freeways and residential neighborhoods, his passenger took off her clothes and orally took control of Martz' other joystick ... if you catch our drift.<br />
<br />
The camera rolled for 45 minutes, though the flight was longer. It begins with Puma topless on the tarmac at Montgomery Field. Puma says the in-flight entertainment was a way to "compensate" Martz for allowing his chopper to be used for a porno shoot in a nearby hangar.<br />
<br />
Martz has had his pilot's license suspended twice and revoked twice, but he's still flying -- at least for now.</p>

			<p>Registration N90943, Clipper Sovereign Of The Skies, was an around the world flight originating in Philadelphia that flew eastbound with several stops before arriving in San Francisco. On the evening of October 15, 1956 it departed Honolulu, Territory of Hawaii on its final leg to San Francisco at 8:26pm HST. After passing the half way point after 1:00am the number 1 engine began overspeeding and could no longer be controlled. The crew decided to cut fuel to the engine. The crew was unable to feather the prop. After the engine shut down the prop began to windmill in the airstream causing excessive drag. The crew calculated with the additional drag they no longer had sufficient fuel to reach San Francisco or return to Honolulu. At this time the United States Coast Guard maintained a ship at Ocean Station November between Hawaii and the California coast. The crew contacted the USCG Pontchartrain and advised them they were making preparations to ditch the aircraft at sea. The plane was flown to the Pontchartrain's location and orbited overhead until daylight. At 6:15am the plane was ditched at sea in sight of the Pontchartrain at 30Â°2'N 140Â°9'W. All 31 on board survived the successful ditching and were rescued by the Coast Guard before the last pieces of wreckage sank at 6:32am.</p>

â€œNot every day we see the biggest passenger airplane in the world land in Iceland, bla bla bla, but today the Airbus 380 was practicing crosswind test landings at Keflavik Airport. Test conditions were 'ideal', with a steady wind blowing 35 to 45 knots.  After only a short stop, the aircraft went up again for repeated approach and test landings that all ended happily. It was amazing to see how this big aircraft handled took on the elements and such strong wind, weighing at about 5 times more than B757 common in Keflavik airport, or 560.000 kilos.  The A380 can take up to 500-600 passengers, bla bla bla.â€<br />

			<p>United Airlines Flight 232 was a scheduled flight operated by United Airlines between Denver and Philadelphia via Chicago. On 19 July 1989, the Douglas DC-10 (Registration N1819U) suffered an uncontained failure of its number 2 engine which destroyed all three of the aircraft's hydraulic systems. With no controls working except the power levers for the two remaining engines, it broke up during an emergency landing on the runway at Sioux City, Iowa, killing 110 of its 285 passengers and one of the 11 crew members.<br />
<br />
Owing to the skill of the crew and a DC-10 instructor pilot, 175 passengers and 10 crew members survived the crash. The disaster is famous within the aviation community as a textbook example of successful Crew Resource Management, due to the effective use of all the resources available aboard the plane for help during the emergency.</p>

It was hijacked by five Pakistan nationals on Christmas Eve, Friday, December 24, 1999, shortly after the aircraft entered Indian airspace at about 5:30 p.m. Indian Standard Time. The hijackers stabbed 25-year-old Rupin Katyal, who later died of his injuries. Ultimately, the plane landed in Afghanistan, where the hijackers agreed to release their hostages in exchange for the release of three Muslim extremists.</p>

			<p>Air Florida Flight 90, an Air Florida flight of a Boeing 737-222 airliner, crashed into the 14th Street Bridge across the Potomac River in Washington, D.C. on January 13, 1982 immediately after takeoff in a severe snowstorm from Washington National Airport in Arlington County, Virginia.<br />
<br />
The aircraft carried 74 passengers and five crew members when it crashed during the failed takeoff attempt. All but five occupants died. The aircraft struck the 14th Street Bridge, which carries Interstate Highway 395 between Washington, D.C., and Arlington County, Virginia. It crushed seven occupied vehicles on the bridge and destroyed 97 feet of guard rail before it plunged through the ice into the Potomac River. The crash occurred less than two miles from the White House and within view of both the Jefferson Memorial and The Pentagon.<br />
<br />
The accident killed 78 people, including four motorists on the 14th Street Bridge. A few survivors were rescued from the icy river by civilians and professionals; President Ronald Reagan commended these acts during his State of the Union speech a few days later. The National Transportation Safety Board determined that the cause of the accident was pilot error. The pilots failed to switch on the engines' internal anti-icing equipment, used reverse thrust in a snow storm prior to take-off, and failed to abort takeoff per FAA regulations, even after detecting a power problem while taxiing and visually identifying ice and snow buildup on the wings.</p>

New York--The Coast Guard and National Transportation Safety Board released Coast Guard footage today of the U.S. Airways passenger plane as it conducted an emergency landing into the Hudson River Jan. 15, 2009.<br />
<br />
The Coast Guard is conducting a safety zone around the plane, which is now located at Battery Park City, N.Y., while a commercial salvage team is working to remove the plane from the water.<br />
The Coast Guard, New York Police and Fire Departments, New York Waterways and Circle Line ferry rescue teams launched a multi-asset response yesterday when the plane ditched onto the Hudson River, at approximately 3:30 p.m.<br />
<br />
Upon initial notification, Coast Guard Sector New York launched a fleet of small rescue boats and the 87-foot Coast Guard Cutter Ridley was diverted to the scene. The Coast Guard Cutter Katherine Walker also arrived on scene to assist in the search and rescue efforts and enforcement of the safety zones.<br />
<br />
Three Coast Guard helicopters from Air Station Atlantic City, N.J., and one from Air Station Cape Cod, Mass., worked closely with New York City Police air assets to provide aerial support.<br />
<br />
The video was captured by Coast Guard Vessel Traffic Service.  The purpose of a VTS is to provide active monitoring and navigational advice for vessels in particularly confined and busy waterways. There are two main types of VTS, surveillance and non-surveillance.<br />
<br />
Surveillance systems consist of one or more land-based sensors<br />
(including radar, Automated Identification Systems and closed circuit television sites), which output their signals to a central location where operators monitor and manage vessel traffic movement. Non-surveillance systems consist of one or more reporting points at which ships are required to report their identity, course, speed, and other data to the monitoring authority.<br />
<br />

			<p>The investigation found that when departing from Tunis on the Tunis-Bari route taken before Flight 1153, the captain found that the plane's fuel level seemed to have increased, but he did not find a refueling slip. The captain took off for Bari without confirming the existence of the refueling slip; there was no slip because the plane was not refueled. In Bari the plane took fuel to a level where the incorrect indicator showed 2,700 kilograms. The correct indicator would have shown 540 kilograms for the Bari-Djerba route. Giuseppe Caldarelli, an investigator, said that the ATR could have reached Palermo with the tail wind. During a two engine flame out crews can feather the propellers to change the angle that the wind hits the propellers, so the plane could glide for a longer period of time. The Tuninter 1153 crew did not feather the propellers. When the second engine failed, the crew did not lower the aircraft's speed to match the ATR 72's optimal gliding speed, in which the aircraft could glide the furthest. A simulator pilot at ATR's facility in France tested the same scenario; in his test he could get the ATR 72 to Palermo by feathering the propellers and reducing the speed to the optimal gliding speed. The simulator pilots were not in imminent danger, and they had chosen to glide. The captain of the ATR did not know that his aircraft was out of fuel and focused on trying to restart the engines; the procedure to do so does not include feathering the propellers. When that did not work, the captain focused on selecting a place to ditch the aircraft. The captain had no instruments and experienced radio interruptions. Before impact Gharbi raised the ATR's nose by nine degrees so that the plane glided across the water surface instead of hitting the ocean head on; this likely allowed the survivors to escape the aircraft. The final investigative report suggested that airlines train their pilots to deal with unusual situations.</p>

			<p>Lubbock, Texas - NewsChannel 11 is continuing to follow a developing situation at Lubbock Preston Smith International Airport, where a plane crashed Tuesday at 4:37 a.m. in a freezing mist.<br />

The airplane is reportedly a FedEx ATR-42, which is a cargo twin-turboprop aircraft. Officials say the plane originated at the Fort Worth Alliance Airport, and came down in Lubbock at the end of the runway, veered off and caught fire.<br />
<br />
Airport officials say the weather was not a factor in the crash.<br />
<br />
Airport first responders are joined by the Lubbock Police and Fire Departments. Firefighters are continuing to monitor hot spots.<br />
<br />
FedEx officials say both pilots are safe, but were taken to University Medical Center for a complete evaluation. <br />
<br />
    * 52 year old male from Portland Oregon is in satisfactory condition.<br />

      26 year old female from Tacoma Washington was treated and released.<br />
<br />
The aircraft is reportedly between the taxiway and a runway.<br />

FedEx flights intended for Lubbock are now being rerouted to Albuquerque, New Mexico.<br />

The airport was closed immediately after the crash, but has since resumed some operations. At least seven flights were delayed, and more are possible.</p>

I takes a lot of money and time to test as thoroughly as the FAA, military, and GE require. More money and time and lives are saved by finding most problems before they become much larger.<br />

Some claim US gas turbine aircraft engines can't take turbulence or odd angles of attack. The claim is clearly false, if you note the shot of the crosswind testing fans. The crosswind fans can simulate hurricane winds at any angle. The fan and the engine can be pivoted to simulate any angle.<br />
<br />
<br />
Typically it is some former soviet block people that make those claims. They think just because they carelessly showboat fighters at air shows doing extreme AOA, that they are the only ones that can do so. The fact is that we don't make a habit of showboating at air shows because it is risky. The former soviet block has crashed a lot of fighters showboating extreme AOAs. The fact is any time you move a gas turbine with reverse airflow your risk of a flameout increases dramatically. The exhaust can recirculate back into the intake the result can be a backfire or flameout or just reduced power; all can be fatal if you are flying with no margin of error at an air show.<br />
<br />
<br />
Normally; US pilots are told to avoid doing things that can cause cavitation or recirculation, particularly wile airborne. They are tested at higher altitude in remote areas to give a good chance of recovery, and restart, and if nothing else ejection.<br />
<br />
There are some exceptions (sometimes turbofans and turboprops), but it still can be a little risky to the engine.</p>

On May 9, 2004, about 1450 Atlantic standard time, Executive Airlines (doing business as American Eagle) flight 5401, an Avions de Transport Regional 72-212, N438AT, skipped once, bounced hard twice, and then crashed at Luis MuÃ±oz Marin International Airport, San Juan, Puerto Rico. The airplane came to a complete stop on a grassy area about 217 feet left of the runway 8 centerline and about 4,317 feet beyond the runway threshold. The captain was seriously injured; the first officer, 2 flight attendants, and 16 of the 22 passengers received minor injuries; and the remaining 6 passengers received no injuries. The airplane was substantially damaged. The airplane was operating under the provisions of 14 Code of Federal Regulations Part 121 as a scheduled passenger flight. Visual meteorological conditions prevailed for the flight, which operated on an instrument flight rules flight plan.<br />

The National Transportation Safety Board determines the probable cause(s) of this accident as follows:<br />
the captain's failure to execute proper techniques to recover from the bounced landings and his subsequent failure to execute a go-around.</p>

"For those who want to fly to London City Airport, a cross wind adds to the excitement (in addition to the short runway and steep approach). The airport itself is located on the river Thames in downtown London, and gives business people very quick access to the city. The Millennium Dome is in the left side of the picture." <br />

"The runway is only 1199 meters long, which is very short in airline runway terms, thus it is only suitable for aircraft with good STOL (short take-off and landing) performance. It also has a 5.5 degree glide slope, as opposed to the conventional 3 degree slope to most runways. The BAE 146 is the main jet type to operate into the place. Most other aircraft are turbo props etc." <br />

"When doing the approach you have to be fully configured, i.e., gear down, full flaps and speed brakes before you commence descent from 3000 feet...or you won't get stopped before reaching the far end of the runway. If it doesn't look or feel right it is generally a good idea to give it away and have another attempt. For whatever reason, this flight crew decided to press, but they DID make the first turn off the runway. If nothing else, it's a testament to the toughness of the BAE 146 airframe. The pilots handled it like a dirtbike!"<br />

Everything is dead right apart from the runway length, which is 1,508 m with a Landing Distance Available ( LDA) of 1,319 m, still quite short. There is also a two pairs of high intensity lights placed either side of the runway at 336 m from the touchdown point. These mark the end of the touchdown zone and a missed approach is required if the aircraft is not down by these lights. I work at LCY and was watching this landing."</p>

			<p>A Continental Airlines plane that lost a wheel in a dramatic touchdown captured on tape by a Toronto-area man has been repaired and yesterday returned to shuttling passengers in New York.<br />

Investigators with the U.S. Federal Aviation Administration and those with the plane's Downsview manufacturer, Bombardier, probed Tuesday's incident at Buffalo International Airport and cleared the Q400 for service.<br />
<br />
The jaw-dropping, 37-second video was obtained by the Toronto Sun from a passenger on the flight, who didn't want to be identified.<br />
<br />
Yesterday, it was aired on major U.S. news outlets, including ABC, NBC and CBS, and was the lead item on the New York Post website.<br />
<br />
The Canadian-made Q400 Bombardier turbo-prop, which has had a recent association with technical glitches, was the aircraft that crashed near Buffalo in February of this year, killing 50 people.<br />
<br />
The video shows Flight 3268 -- with 73 people on board and operated by Colgan Airlines -- landing Tuesday when ashes and debris fell off the wheel as it was lowered. Parts of the landing gear and a right wheel could be seen falling off.<br />
<br />
Audio from the Buffalo air control tower suggested the pilot wasn't aware the wheel had detached until told by a controller, who shut down a runway so the parts could be found by ground workers. The plane was towed to a parking spot.<br />
<br />
FAA spokesman Jim Peters said a hydraulic line leading to a main gear was cut and part of a hub assembly of the landing gear was missing. The parts were found in grass along the runway.<br />
<br />
"This is a straight incident where a tire came off an aircraft," Peters said from New York City yesterday. "Some parts were lost from a hub assembly."<br />
<br />
He said the aircraft was back on its scheduled Newark-to-Buffalo run yesterday.<br />
<br />
Bombardier spokesman John Arnone said the parts that fell off are being sent to the manufacturer to determine what happened.<br />
<br />
"This is a rare and unusual incident," Arnone said. "We are trying to determine the root cause of the incident."<br />
<br />
He said the wheel that came off was recently replaced by technicians.<br />
<br />
Robert Deluce, founder of Toronto's Porter Airlines that has several Q400s in its fleet, called the plane a "safe and reliable" aircraft, with a great safety record.<br />
<br />
"We have the highest maintenance and reliability record of any Q400," Deluce said yesterday. "There is no indication that this is anything other than a localized incident.<br />

"There is no indication that this is a fleet problem."</p>

"I love that video of a DC-3 on a grass strip.  I recognized it right away.  It was built in '42 as a C-47 and sold right after the war as surplus.  It stayed with several companies and then, in the late '70s, was bought by Piedmont Airlines, who had it converted to a DC-3, as seen on the video.  They used it for PR, incentive flights and employee excursions.  Piedmont wanted a DC-3 since that's what they started the airline with.  When US Airways bought them out, of course they didn't want it and put it on the market.  Thanks to the generosity of many people we bought it and still fly it to air shows as shown."</p>

Summary: The plane was scheduled to perform a series of fly-bys at an air show. The plane was to descend to 100 ft. altitude with landing gear and flaps extended. The automatic go-around protection was inhibited for the maneuver. During the maneuver, the plane descended thru 100 ft. to an altitude of 30 feet and hit trees at the end of the runway. The aircraft was totally destroyed by the successive impacts and violent fire which followed. The pilot allowed the aircraft to descend through 100 ft. at slow speed and maximum angle of attack and was late in applying go-around power. Unfamiliarity of the crew with the landing field and lack of planning for the flyby.</p>

"This shows FG flight 709, bound from Kabul to Dubai, which left the Kabul Airport with 261 passengers on board.  After departure the pilot had a problem with the front wheel gear and could not retract it.  Instead of proceeding to Dubai Airport (flight time 2:30 minutes), with all relevant security measures he decided to land back at Kabul Airport." <br />

"Capt. Abdul Rahman asked the tower to investigate the problem.  Kabul Airport sent an ICRC plane to investigate the problem from beneath the aircraft.  After the problem was located, the captain released most of all the full tanks of kerosene and made a final approach at Kabul Airport.  At this time there were still about 30,000 mines lying around beside the apron." <br />

"After the plane came off the runway nothing happened, and the fire brigades went to rescue the aircraft and passengers. The plane came to stand about 50 meters beside the runway without any problems.  All passengers escaped the aircraft safely. Capt. Rahman got out of the plane, had a cigarette smoke, and took all the passengers to Dubai in a second plane approximately an hour later.   ISAF than pulled the plane from beside the apron, stretched the wheel straight, and pulled it to the maintenance department." <br />

Basically, what has been written is correct regarding the height, speed and climbout angle, however it wasn't at an airshow.  It was a private function for past and present members of 40 Squadron RNZAF and at the end of the day there was a small flying display by the 757 and a C130 Hercules. 40 Sqn operate both types of aircraft. For your info: The RNZAF purchased their two 757's from Transavia and we started flying them as soon as they were re-painted and delivered. No modifictions were done to them, so in effect, this display is from a standard 757-200 with RR RB211 engines fitted. Both 757's are now at Mobile, Alabama having some extensive mods done to them - including the fitting of a large cargo door in the front left side, strengthening the forward section floor for loading of freight pallets, fitting of airstairs which operate from the rear entry door (these are really long as the tail end is a long way off the ground) and the fitting of a crew entry ladder.<br />
<br />
The large upwards opening cargo door is a huge job. Normally when 757's get converted to cargo planes, that's all they can carry is the emergency oxygen system, reading lights, air ducts and everything else in the overhead panels are removed in total. In our Mod, we're getting a combi fit. This means there are numerous combinations available to use: part cargo, part passenger in various layouts or a full passenger fit. This means that the overhead panels need to remain - even on the door. So it'll be interesting to see how everything has been wired and plumbed. I had heard talk of getting the engines uprated but can't confirm if this is happening. The first aircraft was recently finished and is now entering the flight test evaluation phase of the project once some smaller niggles have been rectified.<br />

			<p>The Concorde, loaded with fuel for the trans-Atlantic flight, went down shortly after takeoff, crashing near the town of Gonesse, about 10 miles north of Paris, at 4:44 p.m. local time.<br />
All 109 people on board were killed. Three children were among the passengers. Four people in a local hotel on died, with a dozen people on the ground injured.<br />
A French judge ordered Continental Airlines and five people to stand trial for manslaughter in connection with the horrific crash of a Concorde jet that killed 113 people eight years ago in France, a prosecutor said July 2008.<br />
Two of the people to be tried are employees of Continental, the prosecutor in the Paris suburb of Pontoise said in a statement.<br />
<br />
Two others were employed by Aerospatiale, the maker of Concorde and the precursor of plane-maker Airbus. The fifth is an employee of the French civilian aviation authority.<br />
<br />
The No2 main gear wheel tyre is missing after the explosive burst and that un-burnt fuel is flowing from the ruptured tank. A few feet from the tank around the engine area the fuel appears to be being ignited from the fuel that is already burning. The initial cause of the ignition is believed to be from sparks derived from shorted wiring in the landing gear bay area.<br />
<br />
French investigators blamed a titanium strip on the runway from a Continental Airlines DC-10, which took off just before the luxury jet.<br />
<br />
John Taylor, a mechanic who allegedly fitted the non-standard strip is to stand trial for manslaughter. The titanium strip caused one of the Concorde's tires to burst, which sent debris flying into its fuel tanks. Continental's chief of maintenance Stanley Ford will also stand trial for manslaughter.<br />
The French judicial inquiry also determined the tanks lacked sufficient protection from shock and that Concorde's makers had been aware of the problem since 1979.<br />
Former Concorde officials Henri Perrier, 79, and Jacques Herubel, 73, are accused of knowing about problems with the jets and failing to detect and fix them. Claude Frantzen, 71, director of technical services at the civil aviation authority DGAC from 1970 to 1994, was also ordered to stand trial.<br />

Previous to Concorde taking off that fateful day was a continental dc-10 headed for Washington. It was a metal strip from inside one of its engines that was to trigger the catastrophically unfortunate series of events. As Concorde was reaching take off speed it hit this metal strip puncturing the tyre of wheel number two on the port landing assembly. Pieces of rubber then began to ricochet of the undercarriage, soon piercing fuel tank cover 5. A major leak ensues and it is concluded that the kerosene is ignited due to the extreme heat experienced around the wheel assembly. Engines one and two are severely affected by the hot gases emanating from the burning kerosene which are continually being ingested, and they rapidly lose power. The plane is enveloped in flames and sufficient control is lost. The plane crashes and All 109 people on board are killed. Once a conclusion has been reached as to the cause of the incident, appropriate modifications are added such as stronger tyre's which are able to withstand piercing objects at take off speed and in the unlikely occurrence that one does get punctured kevlar sheilds are added to the lining of the fuel tank covers.<br />

			<p>Air Florida Flight 90, an Air Florida flight of a Boeing 737-222 airliner, crashed into the 14th Street Bridge across the Potomac River in Washington, D.C. on January 13, 1982 immediately after takeoff in a severe snowstorm from Washington National Airport in Arlington County, Virginia.<br />
<br />
The aircraft carried 74 passengers and five crew members when it crashed during the failed takeoff attempt. All but five occupants died. The aircraft struck the 14th Street Bridge, which carries Interstate Highway 395 between Washington, D.C., and Arlington County, Virginia. It crushed seven occupied vehicles on the bridge and destroyed 97 feet of guard rail[3] before it plunged through the ice into the Potomac River. The crash occurred less than two miles from the White House and within view of both the Jefferson Memorial and The Pentagon.<br />
<br />
The accident killed 78 people, including four motorists on the 14th Street Bridge. A few survivors were rescued from the icy river by civilians and professionals; President Ronald Reagan commended these acts during his State of the Union speech a few days later. The National Transportation Safety Board determined that the cause of the accident was pilot error. The pilots failed to switch on the engines' internal anti-icing equipment, used reverse thrust in a snow storm prior to take-off, and failed to abort takeoff per FAA regulations, even after detecting a power problem while taxiing and visually identifying ice and snow buildup on the wings.</p>

This three-dimensional (3-D) animated reconstruction shows the last 2 minutes of the February 12, 2009, accident involving a Bombardier DHC-8-400, N200WQ, operated by of Colgan Air, Inc., which crashed about 5 nautical miles northeast of Buffalo-Niagara International Airport, Buffalo, New York, while on an instrument landing system approach to runway 23. During the approach, a pitchup motion occurred, followed by a left roll and then a right roll. During these maneuvers, both the stick shaker and stick pusher were activated, and the speed decreased. After further pitch and roll excursions, the airplane entered a steep descent from which it did not recover.<br />
The animation shows excerpts from the flight data recorder (FDR), the cockpit voice recorder (CVR) transcript, recorded radar data, and aircraft performance data. It does not depict the weather or visibility conditions at the time of the accident. The animation does not include audio.<br />
The upper portion of the animation shows a 3-D model of the airplane and the airplanes motions during the accident sequence. In this area, selected content from the CVR transcript or other annotations are superimposed as text at the time that the event occurred. All times (in eastern standard time) are shown on the right side of the screen.<br />
The lower portion of the animation depicts instruments and indicators, which display selected FDR or calculated parameters. The instruments and indications are shown in three sections, which are (from left to right):<br />

US Airways flight 1549 made an emergency crash-landing into the Hudson River on Thursday, January 15, 2009. All 155 people on board were brought to safety. The miraculous landing took place after the plane struck at least one bird upon takeoff from New York's LaGuardia Airport.<br />

"Regarding the clip about the 'King Air' nose gear failure, that is actually a Beechcraft (now Raytheon) Model 99 operated by AIRPAC Airlines. The incident happened at Boeing field in Seattle Washington on March 28th, 2006."<br />
<br />
"As you can see, the nosewheel separated on landing. The stress on what was left of the gear was too much and it collapsed. At least he got the engines shut down and the props stopped before the collapse. As a result he saved two engines and props."<br />
<br />
"The King Air initially had welded nose gear, then went to a stronger forged one. The King Air and the 99 share a great many parts, but I don't know if the nose gear is one of them. Either way, it appears the nose gear failed due to damage incurred after the failure of either the fork, axle, wheel or retaining nut."</p>

			<p>United Airlines Flight 232 was a scheduled flight operated by United Airlines between Denver and Philadelphia via Chicago. On 19 July 1989, the Douglas DC-10 (Registration N1819U) suffered an uncontained failure of its number 2 engine which destroyed all three of the aircraft's hydraulic systems. With no controls working except the power levers for the two remaining engines, it broke up during an emergency landing on the runway at Sioux City, Iowa, killing 110 of its 285 passengers and one of the 11 crew members.<br />
<br />
Owing to the skill of the crew and a DC-10 instructor pilot, 175 passengers and 10 crew members survived the crash. The disaster is famous within the aviation community as a textbook example of successful Crew Resource Management, due to the effective use of all the resources available aboard the plane for help during the emergency.</p>

			<p>POLK COUNTY -- The Sun 'n Fun Fly-In started Tuesday and already there were some tense moments in the sky for a pilot headed to the event.<br />

The pilot, Kyle Davis, 22, and his passenger, Joe Surowiec, were flying from Winter Haven to Lakeland Sunday when they experienced engine failure.<br />
<br />
Realizing the engine wouldn't cooperate, Davis was able to land the homemade plane safely on Havendale Boulevard.<br />
<br />
"The options for me were land on the road or land on the lake," Davis said.<br />
<br />
News 13 spoke to Eric Norber, a local aviation expert, who explained Davis performed well given the circumstance.<br />
<br />
Norber told News 13 that statistically a plane's engine failure is rare, let alone catching one on camera.<br />
<br />
Davis is a part-time flight instructor with 1,300 hours in the skies. His own flight instructor, John Amundsen, calls his roadway landing, incredible.<br />
<br />
"Watching that video, he was just awesome," Amundsen said. "I just hope, you know, if I ever have a situation like that, I can be as cool as Kyle was."<br />
<br />
Norber said some of the things that could have gone wrong are lack of fuel, fuel contamination and or some sort of engine malfunction.<br />
<br />
He also said that many pilots go their whole lives practicing for this type of situation and never experience one.<br />
<br />
Both men declined on scene medical care. The plane was not damaged either.<br />
<br />
Meanwhile, Sunday's incident didn't scare Davis away from the skies. He plans to fly his own plane to the Sun 'n Fun Fly-In later this week.<br />
<br />
As for finding out what caused the engine failure, Norber said it could be months before the FAA or the NTSB issues a final report on exactly what happened.</p>

			<p>On May 8, 1978 at about 9:20 p.m., a National Airlines 727 with 52 passengers and 6 crew crashed into Escambia Bay on approach to the Pensacola Airport. Three of the passengers drowned while awaiting rescue.<br />
<br />
The National Transportation Safety Board determined the probable cause of this accident was the flightcrew's unprofessionally conducted nonprecision instrument approach; the captain and the crew failed to monitor the descent rate and altitude, and the first officer failed to provide the captain with required altitude and approach performance callouts (each 100 feet below 500 should have been verbally acknowledged). The crew failed to check and utilize all instruments available for altitude awareness, turned off the ground proximity warning system, and failed to configure the aircraft properly and in a timely manner for the approach.<br />
<br />
Contributing to the accident was the radar controller's failure to provide advance notice of the start-descent point which accelerated the pace of the crew's cockpit activities after the passage of the final approach fix .<br />
<br />
At 920:15, the ground proximity warning system (GPWS) whooper sounded, and the " Pull up, pull up" voice warning from the 727 began. The GPWS warning continued until 9:20:24. During this nine second period only two remarks appeared on the CVR transcript: At 9:20:19, Captain George Kunz said "Did you (get) your thing", and at 9:20:21, First Officer (F/O) Leonard Sanderson said, "Descent rate's keeping it up."<br />
<br />
Flight Engineer (F/E) James Stockwell stated that he turned off the GPWS and that he did this in response to what he believed was Captain Kunz's command to turn the system off. He also testified that he believed at the time the aircraft was still above 1,000 feet.<br />
<br />
At 9:20:31, F/O Sanderson said, "I... we're down to fifty feet." Two seconds later, the aircraft hit the water.<br />
<br />
Captain Kunz said that he misread his altimeter at 500 feet and believed he saw 1,500 feet. He stated that "When that figure got on my mind as I ran my scan after that, I was seeing 400 and 300 and they were 14(00) and 13(00) in my mind.  I was looking at the needle instead of looking at the 1,000-foot marker in it. I didn't actually look at the thousand-foot pointer at that time. I just glanced down at the hundred- foot pointer."<br />
<br />
F/O Sanderson said that he failed to make the required altitude callouts, because he was never aware of the fact that the aircraft was below 1,000 feet until just before impact.<br />

			<p>Gol Transportes AÃ©reos Flight 1907 (ICAO: GLO 1907, IATA: G3 1907) was a Boeing 737-8EH SFP, registration PR-GTD, on a scheduled passenger flight from Manaus, Brazil to Rio de Janeiro, which collided in mid-air with an Embraer Legacy business jet on September 29, 2006 over the Brazilian state of Mato Grosso. All 154 passengers and crew aboard the Boeing 737 were killed as the aircraft crashed into an area of dense rainforest, while the Embraer Legacy, despite sustaining serious damage to its left wing and tail, landed safely with its seven occupants uninjured.<br />
<br />
The accident, which triggered a major crisis in Brazilian civil aviation, was the deadliest in that country's aviation history at the time, surpassing VASP Flight 168, which crashed in 1982 with 137 fatalities near Fortaleza. It was subsequently surpassed by TAM Airlines Flight 3054, which crashed on July 17, 2007 with 199 fatalities.<br />
<br />
The accident was investigated by both the Brazilian Air Force CENIPA (Centro de InvestigaÃ§Ã£o e PrevenÃ§Ã£o de Acidentes AeronÃ¡uticos) and the U.S. National Transportation Safety Board (NTSB), with a final report issued on December 10, 2008. CENIPA concluded that the accident was caused by errors committed by both air traffic controllers as well as the Embraer's pilots, while the NTSB determined that all pilots acted properly and were placed on a collision course by a variety of "individual and institutional" air traffic control errors.</p>
